If you are interested in a dog and plan to visit us at one of our adoption shows, please bring this form with you. MS-Word or the Text file (if you don't have MS-word)works best to complete the form. Please fill it in, save to your computer using a different file name (please use the dogs name in the file name) and email it to the foster or to adoptinquiry@cockerspanielrescue.comYou should also print it off and bring it to our adoption shows.
Once you find a dog to rescue, a volunteer from Oldies But Goodies will explain the adoption agreement. Each adopter is required to sign this agreement at the time of adoption.
If you need to give up a dog please fill out this form when you turn in your pet. This will help him keep his name and help us adopt him.
