If you are interested in a dog and plan to visit us at one of our adoption shows, please bring this application with you. The MS Word-fill in document can be saved. Please fill it in, save to your computer (please use the dogs name in the file name) and email it to the foster or to adoptinquiry@cockerspanielrescue.com You should also print it off and bring it to our adoption shows. The Adobe fill in form can be printed out, but not saved.

Once you find a dog to rescue, a volunteer from Oldies But Goodies will explain the adoption agreement. Each adopter is required to sign this agreement at the time of adoption.

If you need to give up a dog please fill out this form when you turn in your pet. This will help him keep his name and help us adopt him.
